Replaced AC_HEADER_TIOCGWINSZ and always just include sys/ioctl.h if it is available - GH#74.

Makefile.in

Rewrote configure.in and replaced header.in with one generated by autoheader, added copyright notice to all source files as per GNU standards, and changed "pv --version" output to conform to GNU standards.
