Fix Coverity warning of q2t_ctio_to_cmd() dead code
Coverity warns that 'handle == Q2T_SKIP_HANDLE' can never occur, because
code preceding the test has masked out a handle bit that would be
required for the test to succeed.

Allow a set of completion vectors to be associated with each InfiniBand
HCA port and allocate the completion vector for each session in a
round-robin fashion from the per-port set. This helps to spread the
InfiniBand interrupt workload over multiple CPU's, at least if different
InfiniBand MSI-X vectors are associated with different CPU's.

Make the SCSI device identification page (83h) EUI-64 and NAA
IDs configurable. If neither the eui64_id nor the naa_id sysfs
attribute has been set, export the first eight bytes of the
t10_dev_id as an EUI-64 ID. If the NAA ID but not the EUI-64 ID
has been set, report the NAA ID only. If both IDs have been set,
report both.

a sysfs attribute 'size' which is the size of a vdisk device in
bytes. Make the size of vdisk_nullio devices configurable. Accept
"size" and "size_mb" as creation parameters for vdisk_nullio
devices. Generate a CAPACITY DATA HAS CHANGED unit attention after size
changes. Refuse any attempt to change the size into a number that is
not a multiple of the block size.

As Mikko Kortelainen <kordex@gmail.com> explained on the SCST IRC
channel, it's not necessary to replace the qla2xxx directory in the
kernel source tree in order to build the SCST qla2x00t target driver.
Also, if neither SCSI pass-through nor iSCSI will be used rebuilding
the kernel tree can be skipped entirely. The patch below updates the
qla2x00t howto accordingly, and also implements the following changes:
- Fix everything the W3C HTML validator complains about:
- Add DOCTYPE declaration.
- Add charset declaration.
- Change </br> into <br> since </br> is invalid.
- Change one occurrence of & into &.
- Use proper HTML constructs for numbered and unnumbered enumerations.
- Use a fixed-sized font and the brown color for all computer output
text (via inline CSS).
- Use hyperlinks to refer to other steps.
- Fix spelling errors.
- Merge the Debian and non-Debian howto's because the differences are
so small that it's not justified to maintain two different documents.
