Bart Van Assche d84fc0783d vdisk_fileio: Add support for asynchronous I/O processing 
Using asynchronous has the advantage that SCSI commands can be processed
concurrently even if only one SCST command thread has been configured.
Asynchronous direct I/O has the additional advantage that the page cache
is bypassed and hence that the command processing overhead is reduced.

Asynchronous I/O is not used when DIF is enabled because it is
nontrivial to ensure that a DIF read received after a DIF write
returns the data that has just been written.

In Linux kernel v4.19 the function scsi_execute() has been replaced by the
following macro:

This macro only accepts a sense buffer that has been declared as an array
but no sense buffers that have been passed as a pointer to a function. To
avoid build errors, convert scst_scsi_execute() from a function into a macro.

Vladislav Bolkhovitin 464b87730c scst: make tgt_dev per-CPU array directly referenced 
Modern distributions come with kernels configured for thousands CPUs
(NR_CPUS), which leads to per-CPU array of SGV pool references in each
tgt_dev exceed tens of KB requiring high order page allocations that are
not reliable on aged systems leading to inabilities to create new
sessions.
