The PRUNE MATERALIZED VIEW statement is performed as follows:
1. Perform a range scan of the view table from the view replicas based
on the ranges specified in the statement.
2. While reading the paged scan above, for each view row perform a read
from all base replicas at the corresponding primary key. If a discrepancy
is detected, delete the row in the view table.
When reading multiple rows, this is very slow because for each view row
we need to performe a single row query on multiple replicas.
In this patch we add an option to speed this up by performing many of the
single base row reads concurrently, at the concurrency specified in the
USING CONCURRENCY clause.
Aside from the unit test, I checked manually on a 3-node cluster with 10M rows, using vnodes. There were actually no ghost rows in the test, but we still had to iterate over all view rows and read the corresponding base rows. And actual ghost rows, if there are any, should be a tiny fraction of all rows. I compared concurrencies 1,2,10,100 and the results were:
* Pruning with concurrency 1 took total 1416 seconds
* Pruning with concurrency 2 took total 731 seconds
* Pruning with concurrency 10 took total 234 seconds
* Pruning with concurrency 100 took total 171 seconds
So after a concurrency of 10 or so we're hitting diminishing returns (at least in this setup). At that point we may be no longer bottlenecked by the reads, but by CPU on the shard that's handling the PRUNE

There is a difference how ScyllaDB and Cassandra handle conditional
batches with different IF statements (such as "IF EXISTS" and "IF NOT
EXISTS").
This commit explicitly documents the differences in the behavior.

Cassandra 4.1 announced a new option to create a role with:
`HASHED PASSWORD`. Example:
```
CREATE ROLE bob WITH HASHED PASSWORD = 'hashed_password';
```
We've already introduced another option following the same
semantics: `SALTED HASH`; example:
```
CREATE ROLE bob WITH SALTED HASH = 'salted_hash';
```
The change hasn't made it to any release yet, so in this commit
we rename it to `HASHED PASSWORD` to be compatible with Cassandra.
Additionally, we adjust existing tests to work against Cassandra too.

We update the relevant articles addressing backing-up
and restoring the schema by specifying that the user
performing it must be a superuser. We also update
the required version of cqlsh.
Additionally, we add an article covering the fundamental
information on `DESCRIBE SCHEMA`.
Bind variables in CQL have two formats: positional (`?`) where a
variable is referred to by its relative position in the statement,
and named (`:var`), where the user is expected to supply a
name->value mapping.
In 19a6e69001 we identified the case where a named bind variable
appears twice in a query, and collapsed it to a single entry in the
statement metadata. Without this, a driver using the named variable
syntax cannot disambiguate which variable is referred to.
However, it turns out that users can use the positional call form
even with the named variable syntax, by using the positional
API of the driver. To support this use case, we add a configuration
variable to disable the same-variable detection.
Because the detection has to happen when the entire statement is
visible, we have to supply the configuration to the parser. We
call it the `dialect` and pass it from all callers. The alternative
would be to add a pre-prepare call similar to fill_prepare_context that
rewrites all expressions in a statement to deduplicate variables.
A unit test is added.

The document docs/cql/cql-extensions.md documents Scylla's extension
of *synchronous* view updates, and mentioned a few cases where view
updates are synchronous even if synchronous updates are not requested
explicitly. But with tablets, these statements and examples are no
longer correct - with tablets, base and view tablets may find
themselves migrated to entirely different nodes. So in this patch
we correct the statements that are no longer accurate.
Note that after this patch we still have in this document, and in
other documents, similar promises about CQL *local secondary indexes*.
Either the documentation or the implementation needs to change in
that case too, but we'll do it in a separate patch.

object storage for a keyspace.
The "Keyspace storage options" section already
existed in the doc, but it was not valid as
the support was only added in version 5.4
The scope of this commit:
- Update the "Keyspace storage options" section.
- Add the information about object storage support
to the Data Definition> CREATE KEYSPACE section
* Marked as "Experimental".
* Excluded from the Enterprise docs with the
.. only:: opensource directive.
This commit must be backported to branch-5.4,
as support for object storage was added
in version 5.4.

The goal is to make the available defaults safe for future use, as they
are often taken from existing config files or documentation verbatim.

Previously, we rejected map subscripts that are NULL, as well as
LIKE patterns that are NULL. General SQL expression evaluation
allows NULL everywhere, and doesn't raise errors - an expression
involving NULL generally yields NULL. Change the behavior to
follow that. Since the new behavior was previously disallowed,
no one should have been relying on it and there is no compatibility
problem.
Update the tests and note it as a CQL extension.
Our handling of NULLs in expressions is different from Cassandra's,
and more uniform. For example, the filter "WHERE x = NULL" is an
error in Cassandra, but supported in Scylla. Let's explain how and why.

It was pointed out to me that our description of the synchronous_updates
materialized-view option does not make it clear enough what is the
default setting, or why a user might want to use this option.
This patch changes the description to (I hope) better address these
issues.
