1d84a254c0 flat_mutation_reader: Split readers by file and remove unnecessary includes.
The flat_mutation_reader files were conflated and contained multiple
readers, which were not strictly necessary. Splitting optimizes both
iterative compilation times, as touching rarely used readers doesn't
recompile large chunks of codebase. Total compilation times are also
improved, as the size of flat_mutation_reader.hh and
flat_mutation_reader_v2.hh have been reduced and those files are
included by many file in the codebase.

With changes

fcb8d040e8 treewide: use Software Package Data Exchange (SPDX) license identifiers
Instead of lengthy blurbs, switch to single-line, machine-readable
standardized (https://spdx.dev) license identifiers. The Linux kernel
switched long ago, so there is strong precedent.

Three cases are handled: AGPL-only, Apache-only, and dual licensed.
For the latter case, I chose (AGPL-3.0-or-later and Apache-2.0),
reasoning that our changes are extensive enough to apply our license.

The changes we applied mechanically with a script, except to
licenses/README.md.

778230f8b8 frozen_mutation: introduce unfreeze_upgrading method
This helper function is similar to the ordinary `unfreeze` of
`frozen_mutation` but in addition to the schema_ptr supplies a
custom column_mapping which is being used when upgrading the
mutation.

Needed for a subsequent patch regarding column mappings history.

16246d1c99 frozen_schema: make freezing constructor explicit
Freezing is an expensive operation, that involves serializing the entire
mutation. Having an implicit freezing constructor means this can happen
as part of an implicit type conversion without the programmer even
noticing, even when this is not really necessary.

ca4a89d239 dht: add dht::decorate_key
and replace all dht::global_partitioner().decorate_key
with dht::decorate_key

It is an improvement because dht::decorate_key takes schema
and uses it to obtain partitioner instead of using global
partitioner as it was before.

f798f724c8 frozen_mutation: Guard against unfreezing using wrong schema
Currently, calling unfreeze() using the wrong version of the schema
results in undefined behavior. That can cause hard-to-debug
problems. Better to throw in such cases.

fa2b0384d2 Replace std::experimental types with C++17 std version.
Replace stdx::optional and stdx::string_view with the C++ std
counterparts.

Some instances of boost::variant were also replaced with std::variant,
namely those that called seastar::visit.

Scylla now requires GCC 8 to compile.

eb357a385d flat_mutation_reader: make timeout opt-out rather than opt-in
Currently timeout is opt-in, that is, all methods that even have it
default it to `db::no_timeout`. This means that ensuring timeout is used
where it should be is completely up to the author and the reviewrs of
the code. As humans are notoriously prone to mistakes this has resulted
in a very inconsistent usage of timeout, many clients of
`flat_mutation_reader` passing the timeout only to some members and only
on certain call sites. This is small wonder considering that some core
operations like `operator()()` only recently received a timeout
parameter and others like `peek()` didn't even have one until this
patch. Both of these methods call `fill_buffer()` which potentially
talks to the lower layers and is supposed to propagate the timeout.
All this makes the `flat_mutation_reader`'s timeout effectively useless.

To make order in this chaos make the timeout parameter a mandatory one
on all `flat_mutation_reader` methods that need it. This ensures that
humans now get a reminder from the compiler when they forget to pass the
timeout. Clients can still opt-out from passing a timeout by passing
`db::no_timeout` (the previous default value) but this will be now
explicit and developers should think before typing it.

There were suprisingly few core call sites to fix up. Where a timeout
was available nearby I propagated it to be able to pass it to the
reader, where I couldn't I passed `db::no_timeout`. Authors of the
latter kind of code (view, streaming and repair are some of the notable
examples) should maybe consider propagating down a timeout if needed.
In the test code (the wast majority of the changes) I just used
`db::no_timeout` everywhere.

Tests: unit(release, debug)

5f08831192 streamed_mutation: fix reversing range tombstones
Right now reversed streamed mutation emits range tombstones after the
mutation fragments affected by them. This breakes the queries.

This patch reworks the way range tombstones are handled in reversed
streams:
 - range tombstones are no longer flipped -- invariant that start bound
   is smaller than the end bound always holds
 - in reversed streams they are ordered by their end_position()

Fixes #2982.

8bb672502d fragment_and_freeze: allow callback to stop iteration
There is a user of fragment_and_freeze() (streaming) that will need
to be able to break the loop Right now, it does that between
streamed_mutation, but that won't be possible after we switch to flat
readers.

38ee69dee0 idl: allow writers to use any output stream
Original IDL generated code was hardcoded to always use bytes_ostream.
This patch makes the output stream a template parameter so that any
valid output stream can be used.
Unfortunately, making IDL writers generic requires updates in the code
that uses them, this is fixed in C++17 which would be able to deduce the
parameter in most cases.

ef57b9a26f rename memory_usage() to external_memory_usage() where applicable
Renaming the function to external_memory_usage() makes it clear that
sizeof(T) is not included, something that was a source of confusion in
the past.

3707d7fec3 frozen_mutation: use bytes_ostream internally
Unlike bytes, bytes_ostream supports fragmented buffers, thus reducing
the pressure on the memory allocator caused by large frozen partitions.

93cc4454a6 streamed_mutation: emit range_tombstones directly
Originally, streamed_mutations guaranteed that emitted tombstones are
disjoint. In order to achieve that two separate objects were produced
for each range tombstone: range_tombstone_begin and range_tombstone_end.

Unfortunately, this forced sstable writer to accumulate all clustering
rows between range_tombstone_begin and range_tombstone_end.

However, since there is no need to write disjoint tombstones to sstables
(see #1153 "Write range tombstones to sstables like Cassandra does") it
is also not necessary for streamed_mutations to produce disjoint range
tombstones.

This patch changes that by making streamed_mutation produce
range_tombstone objects directly.
