these unused includes are identified by clang-include-cleaner.
after auditing the source files, all of the reports have been
confirmed.

assert() is traditionally disabled in release builds, but not in
scylladb. This hasn't caused problems so far, but the latest abseil
release includes a commit [1] that causes a 1000 insn/op regression when
NDEBUG is not defined.
Clearly, we must move towards a build system where NDEBUG is defined in
release builds. But we can't just define it blindly without vetting
all the assert() calls, as some were written with the expectation that
they are enabled in release mode.
To solve the conundrum, change all assert() calls to a new SCYLLA_ASSERT()
macro in utils/assert.hh. This macro is always defined and is not conditional
on NDEBUG, so we can later (after vetting Seastar) enable NDEBUG in release
mode.

Rather than calling on_change for each particular
application_state, pass an endpoint_state::map_type
with all changed states, to be processed as a batch.
In particular, thise allows storage_service::on_change
to update_peer_info once for all changed states.

Now that the endpoint_state isn't change in place
we do not need to copy it to each subscriber.
We can rather just pass the lw_shared_ptr holding
a snapshot of it.

Pass permit_id to subscribers when we acquire one
via lock_endpoint. The subscribers then pass it back to
gossiper for paths that acquire lock_endpoint for
the same endpoint, to detect nested locks when the endpoint
is locked with the same permit_id.

After fcb8d040 ("treewide: use Software Package Data Exchange
(SPDX) license identifiers"), many dual-licensed files were
left with empty comments on top. Remove them to avoid visual
noise.

Instead of lengthy blurbs, switch to single-line, machine-readable
standardized (https://spdx.dev) license identifiers. The Linux kernel
switched long ago, so there is strong precedent.
Three cases are handled: AGPL-only, Apache-only, and dual licensed.
For the latter case, I chose (AGPL-3.0-or-later and Apache-2.0),
reasoning that our changes are extensive enough to apply our license.
The changes we applied mechanically with a script, except to
licenses/README.md.

Move replica-oriented classes to the replica namespace. The main
classes moved are ::database, ::keyspace, and ::table, but a few
ancillary classes are also moved. There are certainly classes that
should be moved but aren't (like distributed_loader) but we have
to start somewhere.
References are adjusted treewide. In many cases, it is obvious that
a call site should not access the replica (but the data_dictionary
instead), but that is left for separate work.
scylla-gdb.py is adjusted to look for both the new and old names.
The database, keyspace, and table classes represent the replica-only
part of the objects after which they are named. Reading from a table
doesn't give you the full data, just the replica's view, and it is not
consistent since reconciliation is applied on the coordinator.
As a first step in acknowledging this, move the related files to
a replica/ subdirectory.
This warning can catch a virtual function that thinks it
overrides another, but doesn't, because the two functions
have different signatures. This isn't very likely since most
of our virtual functions override pure virtuals, but it's
still worth having.
Enable the warning and fix numerous violations.

Many headers don't really need to include database.hh, the include can
be replaced by forward declarations and/or including the actually needed
headers directly. Some headers don't need this include at all.
Each header was verified to be compilable on its own after the change,
by including it into an empty `.cc` file and compiling it. `.cc` files
that used to get `database.hh` through headers that no longer include it
were changed to include it themselves.
Introduce a subscribers_list class that exposes 3 methods:
- push_back(s) - adds a new element s to the back of the list
- remove(s) - removes an element s from the list
- for_each(f) - invoke f on each element of the list
- make a subscriber_list store shared_ptr to a subscriber
to allow removing (currently it stores a naked pointer to the object).
subscribers_list allows push_back() and remove() to be called while
another thread (e.g. seastar::async()) is in the middle of for_each().
