Kefu Chai 3dfb0f86f1 db: add formatter for error_injection_at_startup ...

before this change, we rely on the default-generated fmt::formatter
created from operator<<, but fmt v10 dropped the default-generated
formatter.

in this change, we define formatters for `error_injection_at_startup`,
and drop its operator<<.
