Duarte Nunes 7fb6a74302 combined_mutation_reader: Drop exhausted readers if not in FF mode
Exhausted readers can be fast forwarded, so we have to keep them
around. However, if the current reader is not fast forwardable, then
we can drop those readers and their buffers.
