Fewer uses of size_t in misc.c

* src/misc.c (assign_string_n, quote_copy_string)
(normalize_filename, replace_prefix, remove_any_file)
(blocking_read, wd_alloc, wdcache_count, chdir_arg, chdir_do)
(read_diag_details, struct namebuf, namebuf_name):
Prefer idx_t to size_t.
