Add `write_consistency_levels_disallowed_violations` and `write_consistency_levels_warned_violations` metrics to track violations of write_consistency_levels guardrails. Add `writes_per_consistency_level` to track what CL is used by writes, regardless of the guardrails configuration. Data gathered by this metric can be used to decide whether enabling a particular write consistency level guardrail in a particular existing cluster is safe. Refs: SCYLLADB-259
