Avi Kivity 5e2fd0bbaf test: lib: enhance make_evaluation_inputs() with support for ttls/timestamps ...

While remaining backwards compatible, allow supplying custom timestamp/ttl
with each fake column value.

Note: I tried to use a formatter<> for the new data structure, but
got entangled in a template loop.
