Tomasz Grabiec 20f5d5d1a1 mvcc: partition_snapshot: Introduce migrate() 
Snapshots which outlive the memtable will need to have their
_region and _cleaner references updated.

The snapshot can be destroyed after the memtable when it is queud in
the mutation_cleaner.
