Botond Dénes d65c1523c2 sstables: migrate all malformed_sstable_exception throw sites to throw_malformed_sstable_exception() ...

Replace all direct 'throw malformed_sstable_exception(...)' call sites
with the new throw_malformed_sstable_exception() helper, which respects
the --abort-on-malformed-sstable-error flag.
