Benny Halevy 5b22731f9a flat_mutation_reader: require close 
Make flat_mutation_reader::impl::close pure virtual
so that all implementations are required to implemnt it.

With that, provide a trivial implementation to
all implementations that currently use the default,
trivial close implementation.
